Florida 4-H Agent Responsibility Form (pdf)

Agents should send this form to the State 4-H Events office when they have youth participating in state or national level events. This form provides emergency contact information from the county extension office

Florida 4-H Participation Form

This form should be completed by all participants, youth and adults, in order to participate in Florida 4-H. We suggest you keep the original form on file at your county extension office and send a copy when registering for an event.

Florida 4-H Medication Form (doc) (fillable pdf)

Complete this form for any medication your child will be taking while attending any 4-H event, including non-prescription drugs, lotions, inhalers or any other items.  This form must accompany your child’s medication at camp.  Write your child’s name on a zip lock bag and place this form along with the medication inside the bag.
